Understanding the impact of HLA molecular mismatch in solid organ transplantation: Are we there yet?
Alloimmunity is a complex process, and the likelihood of rejection and/or development of de novo donor-specific HLA antibody (dnDSA) is influenced by a combination of recipient and donor factors. The development of more quantitative measures of immunologic risk, to replace empiric practices, would allow customized immunosuppression and immune monitoring to allow focused interventions to limit alloimmune responses at their earliest inception.
